I accept private pay and Blue Cross and Blue Shield (all traditional and PPO) plans.
If you are planning on using your health insurance, please bring your card to our first meeting.
If you do not have BCBS, but would like insurance reimbursement for sessions, call your insurance company to see what their procedure is for "out of network" providers.
Please complete a consent to treatment form prior to our first appointment. This form contains general information and policies regarding my practice. You may also want to complete a release of information form, which is optional and allows me to communicate with past or current providers.
